FORM.

QUESTIONS 1-3: IDENTIFY. Use this section to identify rental property + tenant.

1. IDENTIFY: Rental. List address number and unit identification

2. IDENTIFY: Tenant. List first + last name of tenant completing this form to request help

3. IDENTIFY: Phone + Email. List phone number + email of the tenant that is completing this form to request help

QUESTIONS 1-3: ISSUE. Use this section to describe the issue that tenant wants help to resolve.

1. ISSUE: Description. List as much information possible about the issue that the tenant wants to resolve

2. ISSUE: Location. List the location of the issue in the house in case it was not already provided in the description

3. ISSUE: Photos. List the photos that tenant has emailed to owner. Photos must be emailed. Photos are required with only exception if there is nothing visual

QUESTIONS 1-3: RESOLUTION. Use this section to help resolve the issue.

1. RESOLUTION: Trouble shoot. List any actions the tenant has already taken to try to trouble shoot the issue + fix it themselves

2. RESOLUTION: Solution. List any solutions the tenant thinks may fix the issue but that the tenant is not capable of doing themselves

3. RESOLUTION: Trade person. List the type of trade person the tenant thinks is needed to resolve the issue

4. RESOLUTION: Request. List the steps or actions the tenant would like the owner to take to resolve the issue
